The covered entity (CE), Charles George Department of Veterans Affairs Medical Center, reported that an employee sent multiple emails that contained the protected health information (PHI) of 1,541 individuals to the wrong recipients. The PHI involved included names, dates of birth, Social Security numbers, diagnoses, and financial information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE revised its policies and procedures and retrained its staff.
