Charles E. Jones DDS PC

The covered entity (CE), Charles E. Jones, DDS dba Jones Family Dental, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 6,493 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. The CE also established a call center for questions or concerns and provided complimentary credit monitoring services. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards, and retrained its staff. OCR provided technical assistance regarding the HIPAA Breach Notification Rule.
