The covered entity (CE), Charles Cole Memorial Hospital dba UPMC Cole, reported that its business associate (BA) was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 7,376 individuals. The ePHI involved included names, dates of birth, and prescription information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the BA strengthened its technical safeguards to better protect its sensitive data.
