The covered entity (CE), Cerebral Medical Group, reported that the protected health information (PHI) of 6,110 individuals was disclosed when its business associate (BA) mailed postcards that included demographic and medical information. The PHI involved included names, addresses, diagnoses, treatment, and medications. The CE notified HHS, affected individuals, the media, and provided substitute notice. In response to the breach, the BA corrected and revised its mailing policies and procedures and retrained all employees.
