The covered entity (CE), Centura Health, reported that an employee was the victim of an email phishing scheme that affected the protected health information (PHI) of 738 individuals. The PHI involved included names, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional technical and administrative safeguards to better protect its PHI.
