The ePHI involved included names, addresses, birthdates, dates of death, Social Security numbers, drivers’ license information, and health insurance information. The CE notified HHS, affected individuals, and the media. Following OCR’s investigation, the CE implemented new technical safeguards and retrained its workforce members.
