Central Minnesota Mental Health Center

The covered entity (CE), Central Minnesota Mental Health Center, reported that multiple employees were the targets of an email phishing scheme that compromised the protected health information (PHI) of 28,725 individuals. The PHI involved included names, dates of birth, addresses, social security numbers, claims information, diagnoses/conditions, lab results, and medications. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented administrative and technical safeguards to better protect its PHI.
