Central Colorado Dermatology

Central Colorado Dermatology, the covered entity (CE), reported that it was the victim of a cyber-attack involving ransomware which affected the electronic protected health information (ePHI) of 4,065 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, drivers’ license numbers, diagnoses, lab results, and medications prescribed. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also offered free credit monitoring services and identity theft protection services to affected individuals. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
