The covered entity (CE), Centerstone of Indiana, Inc., reported that several employees were the subjects of an email phishing attack that compromised the protected health information (PHI) of 1,700 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, claims information, diagnoses and conditions, lab results, and medications. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards. OCR provided technical assistance during the investigation.
