Cedar Springs Hospital

The covered entity (CE), Cedar Springs Hospital, reported that a workforce member of the CE’s licensing agency, the Colorado Department of Public Health & Environment, lost an unencrypted external thumb drive that contained the electronic protected health information (ePHI) of 2,283 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, medication information, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI. OCR provided the CE with technical assistance regarding the HIPAA Security Rule.
