Catholic Health System, the covered entity (CE), reported that a former employee of its business associate (BA) impermissibly accessed the protected health information (PHI) of 12,759 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and diagnoses. The CE notified HHS, affected individuals, the media, and law enforcement.
