Carteret Health Care

The covered entity (CE), Carteret Health Care, reported that the protected health information (PHI) of 1,235 individuals was stolen from an employee’s car. The PHI involved included names, dates of birth, addresses, telephone numbers, employment information, and medical history. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative and technical safeguards, sanctioned the responsible employee, and provided training on protecting sensitive data.
