The covered entity (CE), Carter and Hoff Dentistry, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 7,000 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, x-rays, and diagnoses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards and provided HIPAA related training to its employees.
