Cardiology Associates of Jonesboro

On or about February 18, 2016, the covered entity (CE), Cardiology Associates of Jonesboro, Inc., discovered that its business associate (BA), Document Output Center, LLC, accidentally mailed appointment reminders to incorrect patients due to a software error. The letters disclosed the names, appointment times and, in some cases, appointment types, of approximately 1,669 patients. In response to the incident, the CE worked with the BA to implement a process to check merged files before mailing to make sure they are correct. The CE provided breach notification to HHS, affected individuals, and the media. OCR obtained assurances that the CE implemented the corrective actions listed above.
