Capital Neurological Surgeons

The covered entity (CE), Capital Neurological Surgeons, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 1,769 individuals. The PHI involved included names, dates of birth, drivers’ license and Social Security numbers, state identification numbers, diagnoses, medications, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice to its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional administrative and technical safeguards to better protect PHI.
