Cambridge Healthcare Services, the covered entity (CE), reported that a laptop computer containing the protected health information (PHI) of 10,866 individuals, was stolen from an employee’s vehicle. The PHI involved included names, diagnoses, clinical information, and treatment information. The CE notified HHS, affected individuals, the media, and offered complimentary credit monitoring services to affected individuals. The CE also provided a dedicated telephone number for questions or concerns. In its mitigation efforts, The CE implemented additional technical safeguards and retrained its staff. OCR provided technical assistance to the CE and obtained assurances that the CE implemented the corrective actions noted.
