The covered entity (CE), Builders FirstSource Flex Plan, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 3,763 individuals. The PHI involved included names, addresses, dates of birth, social security numbers and enrollment information. The CE notified HHS, the affected individuals, and the media. The CE also provided substitute notification to the individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
