The covered entity (CE), Bronson Healthcare Group, reported that its Epic system containing protected health information (PHI) was accessed by an employee who did not have authorization. According to the breach report, the breach affected approximately 1,597 individuals. The PHI involved demographic and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the responsible employee, enhanced its access controls, and improved its threat detection solutions to better protect its PHI. OCR provided technical assistance to the CE.
