The covered entity (CE), Brandywine Urology Consultants, reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 131,825 individuals. The ePHI involved included names, Social Security numbers, addresses, drivers’ license numbers, birthdates, claims and financial information, health insurance information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE strengthened its administrative, technical, and security safeguards to better protect its ePHI.
