The covered entity (CE), BioReference Laboratories, Inc., reported that its business associate (BA) inadvertently made the electronic protected health information (ePHI) of 3,334 individuals viewable over the Internet. The ePHI involved included names, addresses, dates of birth, Social Security numbers, and treatment information. As a result of OCR’s investigation, the CE and BA implemented additional administrative and technical safeguards to better protect sensitive data. OCR obtained assurances that the CE and BA implemented the corrective actions noted.
