Benjamin Rose Institute on Aging

The covered entity (CE), Benjamin Rose Institute on Aging, reported that multiple employees were the victims of an email phishing campaign that compromised the electronic protected health information (ePHI) of 1,452 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license information, Social Security numbers, financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. The CE also provided complimentary credit monitoring services. In its mitigation efforts, the CE strengthened its administrative, technical, and security safeguards to better protect its ePHI. OCR provided technical assistance regarding the HIPAA Security Rule.
