Beech Brook

The covered entity (CE), Beech Brook, reported that an employee was the victim of an email phishing scheme that compromised the electronic protected health information (ePHI) of approximately 2,636 individuals. The ePHI involved included names, dates of birth, medical record numbers, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI.
