The covered entity (CE), Bay City Health & Rehabilitation Center, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 2,000 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims and financial information, and other treatment information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
