AZ Plastic Surgery Center

Arizona Plastic Surgery Center, the covered entity (CE), reported that its computer system was hacked by cyber criminals. This breach incident affected 5,524 individuals. The protected health information (PHI) involved included names, dates of birth, drivers’ license numbers, social security numbers, financial information, diagnoses/conditions, and other treatment information. In response to this breach incident, the CE implemented additional technical safeguards. Following OCR’s investigation, the CE revised its privacy and security policies and procedures, implemented security safeguards, and retrained its workforce members.
