Austin Perio Health

The covered entity (CE), Austin Perio Health, reported that an employee was the victim of an email phishing attack that affected the protected health information (PHI) of 4,339 individuals. The PHI involved included names, dates of birth, Social Security numbers, drivers’ license numbers, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided affected individuals with complimentary credit monitoring services. In response to the breach, the CE implemented additional technical safeguards and retrained its staff on email security.
