Austin Pathology

Austin Pathology, the covered entity (CE), reported that its business associate was the subject of a data security incident affecting the electronic protected health information (ePHI) of 45,429 of its patients. The ePHI involved included names, addresses, dates of birth, credit card and banking information, and dates of treatment. In response to this breach incident, the CE provided notice to OCR, affected individuals, and the media. During the investigation, the CE provided evidence it had a business associate agreement and subsequently, terminated its business relationship.
