August Foot & Ankle, P.C., the covered entity (CE), reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 4,854 individuals. The ePHI involved included names, dates of birth, Social Security numbers, drivers’ license numbers, addresses, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
