The covered entity (CE), Audiology Services Company USA, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 3,917 individuals. The ePHI involved included names, addresses, dates of birth, gender, telephone numbers, health insurance information, clinical information, and Social Security numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical safeguards and retrained its staff on the proper methods of identifying fraudulent email communications. OCR obtained assurances that the CE implemented the corrective actions noted.
