The covered entity (CE), Atrium Health, reported that its historical use of web tracking technology on its patient portal may have resulted in a breach. The protected health information (PHI) of potentially 585,959 individuals was affected by the breach. The PHI involved clinical and demographic information. The CE notified HHS, the affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative safeguards.
