The PHI involved included names, addresses, dates of birth, Social Security numbers, financial information, and other treatment information. The CE notified HHS, the affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI and retrained its workforce members on email security precautions.
