Associated Ophthalmologists of Kansas City

The covered entity (CE), Associated Ophthalmologists of Kansas City, reported that its business associate (BA) was the victim of a cyber-attack that compromised the protected health information (PHI) of 13,461 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, and medication information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services to those affected.
