Associated Dentists-Roseville Office

Associated Dentists – Roseville Office, the covered entity (CE), reported that its office was burglarized and two laptop computers containing the protected health information (PHI) of 4,725 individuals, were stolen. The PHI involved included names, addresses, Social Security numbers, and dates of birth. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In addition, the CE offered complimentary identity theft and restoration services to affected individuals. In its mitigation efforts, the CE implemented additional administrative and technical safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
