Ascension St. Vincent's Coastal Cardiology

The covered entity (CE), Ascension St. Vincent’s Coastal Cardiology, reported that it experienced a cybersecurity incident that affected the protected health information (PHI) of 71,227 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, claims information, diagnoses, lab results and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards.
