The covered entity (CE), Ascension – Our Lady of Lourdes Memorial Hospital, Inc., reported that a business associate (BA) was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 1,745 individuals. The ePHI involved included names, dates of birth, health insurance information, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the BA strengthened its administrative, technical, and security safeguards and retrained its staff.
