Ascension Health Services LLC d/b/a Alpha Wellness & Alpha Medical Centre, the covered entity (CE) reported that it experienced a hacking attack that affected the protected health information (PHI) of 1,714 individuals. The PHI involved clinical, demographic, and treatment information. The CE notified HHS, impacted individuals, and the media. OCR provided technical assistance to the CE regarding the HIPAA Rules. As of April 18, 2025, the CE ceased operations and is no longer in business.
