Ascension Eastwood Clinics

The covered entity (CE), Ascension Eastwood Clinics, reported that an employee transmitted a mass email without utilizing the blind carbon copy function. The email contained the electronic protected health information (ePHI) of approximately 999 individuals. The ePHI involved included names, diagnoses, and email addresses. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the responsible workforce member and trained its staff on email security.
