Armada Physical Therapy of Albuquerque

The covered entity (CE), Armada Physical Therapy of Albuquerque, LLC, reported that a desktop server that contained the electronic protected health information (ePHI) of 1,850 individuals, was stolen. The ePHI involved included names, addresses, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional physical and technical safeguards to better protect its data and equipment.
