Arizona Pain and Spine Institute

The covered entity (CE), Arizona Pain and Spine Institute, reported that a laptop was stolen from an employee’s vehicle that contained the electronic protected health information (ePHI) of 530 individuals. The ePHI involved included names, dates of birth, and ultrasound images. The CE notified HHS, affected individuals, the media, local law enforcement, and provided affected individuals with free credit monitoring services. In its mitigation efforts, the CE adopted additional administrative and security safeguards and sanctioned the responsible employee. OCR provided the CE with technical assistance regarding its security management processes.
