The covered entity (CE), The Arbors Operator LLC, reported that its business associate (BA) experienced a hacking attack that compromised the protected health information (PHI) of 2,808 individuals. The PHI involved included names, dates of birth, Social Security numbers, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE and BA provided complimentary credit monitoring services and implemented additional technical safeguards.
