Apple Blossom Family Practice

The covered entity (CE), Apple Blossom Family Practice, reported that its business associate (BA) was the victim of a ransomware attack affecting the electronic protected health information (ePHI) of 12,053 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses/conditions, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. The CE also provided affected individuals with complimentary identity theft protection services. Following the breach, the CE closed its business and OCR subsequently closed the investigation.
