Apex Laboratory

The covered entity (CE), Apex Laboratory, reported that it was the victim of a cyber-attack affecting the protected health information (PHI) of 7,102 individuals. The PHI involved included names, dates of birth, test results, Social Security numbers, and phone numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards, revised its policies and procedures, and retrained its employees. Following the breach, the CE informed OCR that it is no longer in business.
