Anwan Wellness

The covered entity (CE), Anwan Wellness, LLC, reported that it lost access to one of its electronic medical records systems after a terminated employee changed the administration settings. This breach affected the protected health information (PHI) of 530 patients. The PHI involved included names, addresses, dates of birth, email addresses, and telephone numbers. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards, and retrained its staff.
