Anesthesia Associates of Kansas City

The covered entity (CE), Anesthesiology Associates of Kansas City (AAKC), reported that an employee’s vehicle was burglarized and documents containing the protected health information (PHI) of 3,472 individuals was stolen. The PHI involved included names, dates of birth, and clinical information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also established a complimentary toll free number for questions or concerns. In its mitigation efforts, AAKC sanctioned the responsible employee and implemented administrative safeguards and retrained its employees. OCR obtained assurances that the CE implemented the corrective actions noted.
