Andrews Braces

Andrew Braces, LLC, the covered entity (CE), reported that it was the victim of a cyber-attack involving the electronic protected health information (ePHI) of approximately 16,622 individuals. The ePHI involved included names, Social Security numbers, addresses, birthdates, diagnoses/conditions, medications prescribed, treatment information, and claims information. The CE notified HHS, affected individuals, and the media. Following OCR’s investigation, the CE revised its policy and procedures, retrained its workforce members, and implemented additional technical safeguards.
