AMITA Health Medical Group Primary Care- Hoffman Estates

The covered entity (CE), AMITA Health Medical Group, reported that an employee transmitted a mass email to patients without utilizing the blind carbon copy function. This breach affected the protected health information (PHI) of approximately 1,647 individuals. The PHI involved included names and email addresses. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE sanctioned and retrained the responsible employees.
