The business associate (BA), Amerigroup Iowa, Inc., reported that an employee mailed the protected health information (PHI) of 1,023 individuals to the wrong recipients. The PHI involved included names, dates of birth, health insurance information, claims information, and diagnoses and procedures codes. The BA notified HHS, the covered entity, and the affected individuals. In response to the breach, the BA implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect PHI.
