Allies for Health + Wellbeing

Allies for Health + Wellbeing, the covered entity (CE), reported that an employee was the subject of an email phishing scheme that affected the protected health information (PHI) of 680 individuals. The PHI involved included names, addresses, dates of birth, phone numbers, diagnoses/conditions, lab results, and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice of the breach on its website. In its mitigation efforts, the CE implemented additional technical safeguards to better protect PHI and provided enhanced training to all employees.
