Allegheny County, the covered entity (CE), reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information (PHI) of 689,686 individuals. The PHI involved included names, addresses, phone numbers, Social Security numbers, dates of birth, drivers’ license numbers, diagnoses, claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the incident, the CE and BA strengthened its administrative, technical, and security safeguards to better protect PHI.
