Alive Hospice

Alive Hospice, the covered entity (CE), reported that an employee inadvertently mailed documents containing the protected health information (PHI) of 533 individuals to the wrong recipients. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE sanctioned the responsible employee and implemented additional administrative safeguards to prevent future errors of this nature. OCR obtained assurances that the CE implemented the corrective actions noted.
