The covered entity (CE), Airway Oxygen, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of approximately 500,000 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services to those affected and implemented additional administrative and technical safeguards.
